Harriet’s Story
Inquiry
We received a call from Harriet's primary care physician. He reported a growing concern that Harriet's memory, appearance and personal hygiene had deteriorated over the past several months. Harriet lived alone and he was worried she was not taking her medication as prescribed. Harriet had been diagnosed with diabetes, hypertension and a thyroid disorder.
Aging Well Consult
Nancy and Mary met with Harriet in her home. She agreed to a visit to have her blood pressure checked by two community based nurses. Upon arrival, we found evidence of spoiled food in the refrigerator, an unkempt home with mail and unpaid bills scattered about the kitchen and medication refills that were untouched. After an interview to establish trust, we contacted Harriet's sister to report our concerns.
Plan
With Harriet and her sister's approval, caregivers were introduced by Nancy to create a daily structure with predictable times for meals, medication, showers and activity. The refrigerator and kitchen were cleared of clutter and spoiled food. Harriet and her caregivers began a routine of daily walks in the neighborhood to improved balance and coordination. Sewing cards provided a quiet activity for Harriet in order to experience success.
Outcome
Harriet began to take her medication as prescribed. Her diabetes and blood pressure were in better control. Harriet's hygiene improved with regular showers, clean clothes and daily attention to personal care. We report regularly to Harriet's sister and visit twice a month to monitor and modify the plan of care. Aging Well staff accompanies Harriet to all medical appointments to ensure medical needs are addressed. Harriet is able to remain in her home safely.
