Bea's Story
Inquiry
We received a call from Bea's son who lived in the Greater Boston area. Bea had been diagnosed with Parkinson's disease and dementia. Since her diagnosis, Bea was living out of state in the home of her adult daughter. In the spring of 2007, Bea fell, was hospitalized and eventually transferred to a rehabilitation facility to address her mobility issues. Her children decided she was no longer safe to remain unsupervised in her daughter's home.
Aging Well Consult
Mary and Nancy met with Bea's adult children in her son's home. They requested assistance with finding an appropriate local placement for their Mom which would address her changing needs. They also asked for assistance securing appropriate referrals to local physicians.
Plan
Mary and Nancy contacted Bea's rehabilitation center to catalogue Bea's needs and level of cognitive impairment. They explored local facilities to secure a placement which would most effectively address Bea's comprehensive needs. They made arrangements for a tour and interview with Bea's son. They worked with Bea's daughter to gather familiar objects and send them to Bea's new home. With the approval of Bea's son, medical appointments were made to introduce Bea to her new medical team. Contact was made with Bea's former physicians to have medical records sent to new team to ensure continuity of care.
Outcome
Bea was successfully transferred to her new home. Mary accompanied Bea to her first medical appointments to ensure all issues and needs were addressed. Bea quickly adapted to her new home in Massachusetts.
