Meeting the needs of our clients and families

Older adults often have complex medical and social needs. They require different approaches to medication management, assessment and diagnosis.

At Aging Well, we start our process with a comprehensive initial assessment. This takes the form of a reassuring conversation with the elder family member while we review the living arrangements to determine if the client is able to perform daily tasks unaided.

We thoroughly catalogue any medical or social difficulties and where necessary, consult with the care recipient's physician, social worker, hospital, or nursing facility. Our set of recommendations and individualized care plans are designed to help the client and their family better manage daily needs.

A unique blend

Our unique blend of knowledge and hands on nursing experience means that we are able to offer help that ranges from Alzheimer's facility placement to recommending and supervising health aides and companions. We view ourselves as part of a team that has the client and their family at the center. We work towards a collective solution, whether within the home or at an external facility.

The goal

While our principals tailor our services to uniquely meet the needs of the individual client, the goal is always the same: to seek an outcome that will allow the individual to remain an integral part of their community while remaining safely at home where possible.
